All applicants must:

• Be 27 weeks pregnant or less

• Earn $44,000 or less annually

This program will serve two distinct cohorts:

• One for mothers 18 to 22 years of age

• One for mothers 18 years or older who have served or are serving in the U.S. Armed Forces, the National Guard, or the Reserves

All applicants must:

• Live in one of the following ZIP codes:

  • 21225 — Cherry Hill

  • 21230 — Cherry Hill

  • 21215 — Park Heights

  • 21740 — Hagerstown

• Be at least 18 years of age

• Be 27 weeks pregnant or less

• Be eligible for Medicaid

No SSN or ITIN required

All applicants must:

• Live in one of the ARC-designated counties within the states of Kentucky, Ohio, West Virginia, or Tennessee

• Be at least 18 years of age

• Be at most 14 weeks pregnant

• Submit proof of pregnancy that includes:

  • Patient's name

  • Clinic's name, location, and contact info

  • Date of visit to medical provider

  • Expected due date
